Global Pharmacy Market Outlook, 2031

The Global Pharmacy Market is anticipated to grow, driven by the increasing demand for prescription and over-the-counter medications and the expansion of pharmacy services.

The global pharmacy industry covers the businesses that dispense prescription medicines and sell over-the-counter medicines, health products and personal care items to patients and consumers, together with related services such as vaccinations, medication reviews, testing, minor ailment treatment and chronic disease management. It includes retail pharmacy chains, independent community pharmacies, hospital pharmacies, specialty pharmacies, mail-order and online pharmacies, and pharmacies within supermarkets and mass retailers. Major chains include CVS Health, Walgreens, now privately owned by Sycamore Partners, Walmart and Kroger in the United States; Boots, owned by Walgreens, in the UK; Shoppers Drug Mart in Canada; Chemist Warehouse in Australia; Matsumoto Kiyoshi, Welcia and Tsuruha in Japan; Watsons across Asia; Apollo Pharmacy, MedPlus and Tata 1mg in India; Dasheng, Yifeng and Laobaixing in China; and Raia Drogasil in Brazil. Online players include Amazon Pharmacy, Mark Cuban Cost Plus Drug Company, DocMorris and Redcare in Europe, and JD Health and Alibaba Health in China. In the United States, pharmacy is closely linked to pharmacy benefit managers, including CVS Caremark, Express Scripts owned by Cigna and Optum Rx owned by UnitedHealth, which control networks, reimbursement and much of the specialty and mail-order market. Pharmaceutical wholesalers such as McKesson, Cencora and Cardinal Health supply most pharmacies.

Pharmacies played a central role during the pandemic, administering hundreds of millions of COVID-19 tests and vaccines and demonstrating their value as accessible healthcare providers. Since then, the sector has faced intense financial pressure, particularly in the United States, where low reimbursement from pharmacy benefit managers, falling front-store sales, staffing shortages, opioid litigation settlements and theft reduced margins. Thousands of pharmacies have closed. Rite Aid filed for bankruptcy a second time in May 2025 and closed all its stores, CVS closed hundreds of locations, and Walgreens, after years of losses, was taken private by Sycamore Partners in August 2025 in a deal worth up to about USD 23.7 billion including debt. Independent pharmacies have also closed at a high rate, creating pharmacy deserts in some communities. Policy has focused on pharmacy benefit managers: the US Federal Trade Commission sued the three largest PBMs over insulin pricing in September 2024, states passed laws restricting PBM practices, and in February 2026 Congress enacted federal PBM reform, delinking PBM compensation from drug prices in Medicare Part D, requiring rebate pass-through and supporting fair reimbursement for pharmacies from 2028. Direct-to-consumer drug sales, including TrumpRx.gov from February 2026 and manufacturer platforms for obesity drugs, have created new channels. In other markets, pharmacy chains in India, China, Southeast Asia and Latin America have expanded rapidly, and online pharmacy has grown strongly in Germany, China and India.

Market Dynamics Market Drivers • Rising Prescription Volumes from Ageing and Chronic DiseaseAgeing populations and rising rates of chronic conditions such as diabetes, hypertension, high cholesterol, obesity and depression increase the number of prescriptions dispensed. Older adults take multiple medicines and need regular refills, and the growth of GLP-1 obesity and diabetes drugs has added significant prescription volume and revenue for pharmacies. Expanding insurance coverage in emerging markets brings more people into formal pharmacy channels.
• Expansion of Pharmacy Clinical ServicesGovernments and payers increasingly use pharmacists to relieve pressure on doctors and hospitals. Pharmacists administer vaccines, provide medication reviews, test and treat common infections, prescribe for minor ailments, manage chronic diseases and offer contraception and smoking cessation services. The UK's Pharmacy First scheme, launched in January 2024, allows pharmacists to treat seven common conditions, and Canada, Australia and US states have expanded pharmacists' prescribing powers, creating new revenue streams.

Market Challenges • Reimbursement Pressure and Store ClosuresPharmacy margins on prescriptions have been squeezed by pharmacy benefit managers, government reimbursement cuts and generic price deflation. In the United States, many pharmacies say they are paid less than the cost of some drugs, and thousands of pharmacies closed between 2022 and 2025, including all Rite Aid stores. Front-store sales have declined as consumers buy more online and from mass retailers, and theft and labour costs have increased.
• Staffing Shortages and WorkloadPharmacies face shortages of pharmacists and pharmacy technicians, leading to reduced opening hours, burnout and walkouts by staff at US chains in 2023. Expanding clinical services increases workloads without always providing adequate payment. Drug shortages, including for ADHD stimulants, antibiotics and cancer drugs, create extra work and frustration for pharmacists and patients.

Market Trends • PBM Reform and Changing Pharmacy EconomicsRegulators and legislators have targeted pharmacy benefit managers' practices, including spread pricing, rebate retention and steering patients to affiliated pharmacies. Federal PBM reform enacted in February 2026, state laws, FTC scrutiny and employer demands for transparency are pushing toward cost-plus and pass-through pricing models. Pharmacies are also exploring new reimbursement approaches, such as fees for clinical services and transparent acquisition-cost-based pricing.
• Online, Direct-to-Consumer and Automated PharmacyOnline pharmacies, home delivery and telehealth-linked pharmacies have grown, including Amazon Pharmacy, Cost Plus Drugs, manufacturer direct-to-consumer platforms and TrumpRx.gov in the United States, e-prescription-driven online pharmacies in Germany, and large e-pharmacy platforms in China and India. Pharmacies are adopting central-fill automation, robotic dispensing and AI tools to reduce costs, free pharmacists for clinical work and compete on convenience.

Segment Analysis Retail pharmacies account for the largest share of the pharmacy market, while online and mail-order pharmacies and specialty pharmacies are the fastest-growing pharmacy types. Retail pharmacies, including national and regional chains, independent community pharmacies and pharmacies in supermarkets and mass retailers, account for the largest share of prescriptions dispensed and pharmacy revenue worldwide. They serve as accessible healthcare locations, providing medicines, advice, vaccinations and health products. Chains have consolidated in many markets, while independents remain important in rural areas and in countries with ownership restrictions such as Germany, France and Spain, where only pharmacists may own pharmacies. Hospital pharmacies supply medicines to inpatients and outpatients within hospitals, manage complex drugs such as chemotherapy and infused biologics, and in many countries dispense specialty drugs for home use. Specialty pharmacies handle high-cost medicines for cancer, immunology, rare diseases, HIV and multiple sclerosis, providing patient support and handling complex reimbursement, and in the United States specialty drugs account for about half of pharmacy drug spending. Mail-order and online pharmacies deliver prescriptions to homes and have grown strongly, supported by e-prescriptions, telehealth and consumer demand for convenience. Long-term care pharmacies serve nursing homes and assisted living facilities. Clinic and physician-dispensing pharmacies and compounding pharmacies form smaller segments.

Prescription medicines account for the largest share of pharmacy revenue, while over-the-counter medicines, health and wellness products and pharmacy services provide important additional income. Prescription medicines, including branded drugs, generics and specialty drugs, generate the majority of pharmacy revenue, although margins vary widely, with generics often providing higher margins than expensive branded drugs. GLP-1 drugs for diabetes and obesity have significantly increased prescription revenue in recent years, although margins on these high-priced drugs are thin for many pharmacies. Over-the-counter medicines, including pain relievers, cough and cold products, allergy medicines, digestive remedies and smoking cessation aids, are sold without prescription and offer higher margins, although they face competition from supermarkets and online retailers. Health and wellness products, including vitamins, supplements, medical devices, diagnostic tests, first aid and mobility aids, are growing with consumer interest in self-care. Personal care and beauty products, including skincare, cosmetics and toiletries, form a large part of front-store sales for chains such as Boots, Watsons and Shoppers Drug Mart. Pharmacy services, including vaccinations, medication management, health screenings, point-of-care testing, minor ailment consultations and contraception services, are a growing source of revenue. Convenience items, snacks and household goods make up the rest of front-store sales in some markets.

Chain pharmacies account for the largest share of pharmacy revenue in the United States, the UK and many Asian markets, while independent pharmacies remain significant in continental Europe, India and rural areas. Chain pharmacies, including large national and regional operators, benefit from scale in purchasing, technology, marketing and payer negotiations, and they dominate the pharmacy market in the United States, the UK, Canada, Australia, Japan and several Asian and Latin American countries. Consolidation has continued, although the largest US chains have closed thousands of stores and Walgreens was taken private in 2025. Independent pharmacies, owned by individual pharmacists or small groups, are important for access in rural and underserved areas and have strong relationships with local communities, but they face financial pressure from low reimbursement, and many have closed. In countries with pharmacist ownership rules, such as Germany, France, Italy and Spain, independents remain the norm, often joined through purchasing and marketing groups. In India, most of the country's more than 800,000 pharmacies are independent, though organised chains such as Apollo Pharmacy and MedPlus are growing. In China, chains such as Dasheng, Yifeng and Laobaixing are consolidating a fragmented market. Supermarket and mass merchant pharmacies, such as Walmart, Costco and Kroger, compete on price and convenience. Health system-owned pharmacies have grown in the United States as hospitals expand outpatient and specialty pharmacy operations.

Commercial insurance and government programmes fund most prescription purchases in high-income countries, while out-of-pocket payments dominate pharmacy spending in many emerging markets. Government programmes, including national health services, social insurance systems and US programmes such as Medicare Part D and Medicaid, fund a large share of prescriptions dispensed in pharmacies, and their reimbursement policies strongly affect pharmacy economics. In Europe, pharmacies are reimbursed by national systems under regulated margins and fees. Commercial insurance, including employer-sponsored plans managed by pharmacy benefit managers, funds most prescriptions for working-age Americans, and PBM reimbursement and network decisions determine which pharmacies patients use. Out-of-pocket payments, where patients pay directly, account for most pharmacy spending in India, much of Southeast Asia, Africa and Latin America, making price competition and generic availability important. Cash-pay purchases have also grown in the United States, including discount cards such as GoodRx, cost-plus pricing models and manufacturer direct-to-consumer offers for obesity drugs and other medicines, including through TrumpRx.gov. Copayments and deductibles add to patient costs in insured markets. Employer and workplace health programmes, and government schemes such as India's Jan Aushadhi generic medicine stores, provide low-cost options for patients.

Regional Analysis North America accounts for the largest share of the pharmacy market because of high US drug prices and prescription volumes, while Asia-Pacific is the fastest-growing region as pharmacy chains and online pharmacies expand in China, India and Southeast Asia. North America, led by the United States, accounts for the largest share of pharmacy revenue, reflecting high drug prices, large prescription volumes and a large specialty pharmacy market. The US pharmacy sector has undergone major restructuring, including thousands of store closures, Rite Aid's closure in 2025, Walgreens' acquisition by Sycamore Partners in August 2025 and CVS's store closures, while Amazon Pharmacy, Cost Plus Drugs and direct-to-consumer platforms have grown. Federal PBM reform enacted in February 2026, FTC action against PBMs and state laws such as Arkansas's attempt to ban PBM-owned pharmacies are reshaping pharmacy economics. Canada has large chains such as Shoppers Drug Mart and expanding pharmacist prescribing. Europe is the second largest region, with regulated pharmacy markets, pharmacist ownership rules in many countries and growing online pharmacy, particularly in Germany after the introduction of electronic prescriptions in 2024. The UK's Pharmacy First service expanded clinical roles, but many English pharmacies have closed due to funding pressure. Asia-Pacific is the fastest-growing region. China has a large and consolidating pharmacy market with strong growth in online pharmacy through JD Health and Alibaba Health, Japan has large drugstore chains that combine pharmacy with cosmetics and daily goods, and India's pharmacy market is expanding with organised chains, e-pharmacies and government generic stores. Southeast Asia and Australia have growing chains such as Watsons, Guardian and Chemist Warehouse. Latin America, led by Brazil's Raia Drogasil and Mexico's Farmacias Similares and Farmacias Guadalajara, has large chains, and the Middle East and Africa have growing pharmacy markets.
