Global Cephalosporin Drugs Market Outlook, 2031

The Global Cephalosporin Drugs Market is anticipated to grow, driven by the increasing prevalence of bacterial infections and the continued demand for effective antibiotic treatmen

The global cephalosporin drugs industry covers a large family of beta-lactam antibiotics derived from the fungus Cephalosporium, used to treat bacterial infections of the respiratory tract, urinary tract, skin and soft tissues, bones and joints, bloodstream, abdomen and central nervous system, as well as gonorrhoea and surgical prophylaxis. Cephalosporins are grouped into generations based on their spectrum of activity: first-generation drugs such as cefazolin and cephalexin mainly target Gram-positive bacteria and are widely used for skin infections and surgical prophylaxis; second-generation drugs such as cefuroxime and cefaclor add some Gram-negative activity; third-generation drugs such as ceftriaxone, cefotaxime, ceftazidime, cefixime and cefdinir have broad Gram-negative activity and are among the most widely used antibiotics in hospitals; fourth-generation cefepime is used for serious hospital infections; fifth-generation ceftaroline and ceftobiprole are active against MRSA; and newer agents include the siderophore cephalosporin cefiderocol and cephalosporin and beta-lactamase inhibitor combinations such as ceftazidime-avibactam, ceftolozane-tazobactam, cefepime-enmetazobactam and cefepime-zidebactam. Most cephalosporins are generics produced by manufacturers in India and China, including Aurobindo, Orchid Pharma, Lupin, Cipla, Alkem, Sun Pharma, CSPC and Kelun, and by Sandoz, Fresenius Kabi, Hikma and others, while branded innovators include Shionogi, Pfizer, Merck, Innoviva, Allecra, Basilea and Wockhardt.

Cephalosporins are among the most prescribed antibiotics worldwide, but the market is shaped by rising antimicrobial resistance, generic price competition and supply chain concentration. Resistance through extended-spectrum beta-lactamases, AmpC enzymes and carbapenemases has reduced the effectiveness of older cephalosporins against Enterobacterales and Pseudomonas, driving development of new combinations. Several new cephalosporin-based products have reached the market since 2024. The FDA approved Allecra's Exblifep (cefepime-enmetazobactam) in February 2024 and Basilea and Innoviva's Zevtera (ceftobiprole) in April 2024 for Staphylococcus aureus bacteraemia and other infections, while Venatorx's cefepime-taniborbactam received a complete response letter in February 2024 because of manufacturing questions. In May 2026, the FDA approved Wockhardt's Zaynich (cefepime-zidebactam) for complicated urinary tract infections after it showed a composite cure rate of 89% versus 68.4% for meropenem in the ENHANCE-1 trial, making it the first new chemical entity fully discovered and developed by an Indian company to win FDA approval, following Indian approval days earlier. Ceftriaxone remains the standard treatment for gonorrhoea, but resistant strains have emerged, contributing to the December 2025 approvals of new oral gonorrhoea drugs. Supply problems, including shortages of cefazolin and other injectable cephalosporins in several countries, have highlighted dependence on a small number of manufacturers of active ingredients in China and India.

Market Dynamics Market Drivers • High Burden of Bacterial Infections and Broad Clinical UseCephalosporins are used for a wide range of common and serious infections, including pneumonia, urinary tract infections, skin infections, meningitis, sepsis and gonorrhoea, and for surgical prophylaxis before many operations. Their broad spectrum, safety and familiarity make them first-line choices in many guidelines. Growing surgical volumes, ageing populations and high infection rates in emerging markets support steady demand.
• Need for New Agents Against Resistant BacteriaResistance to older cephalosporins and carbapenems among Gram-negative bacteria has created demand for new cephalosporin-based combinations and novel agents such as cefiderocol and cefepime-zidebactam. Hospitals need options for multidrug-resistant infections, and regulatory incentives such as QIDP designation, priority review and pull incentives in the UK and other countries support development of new cephalosporins.

Market Challenges • Antimicrobial Resistance and Stewardship RestrictionsOveruse of broad-spectrum cephalosporins, particularly third-generation agents, contributes to resistance and to Clostridioides difficile infection, and stewardship programmes restrict their use. The WHO AWaRe classification places many cephalosporins in the Watch group, encouraging countries to reduce their use relative to Access antibiotics. Newer agents are reserved for resistant infections, limiting their sales volume.
• Generic Price Erosion and Supply ConcentrationMost cephalosporins are low-priced generics, and intense competition, tenders and China's volume-based procurement have pushed prices down, reducing manufacturers' margins and leading some to exit. Production of cephalosporin active ingredients and intermediates is concentrated in China and India, and dedicated facilities are needed because of allergy risks, making supply vulnerable to disruptions and shortages.

Market Trends • Novel Cephalosporin and Beta-Lactamase Inhibitor CombinationsCompanies are pairing cephalosporins with new beta-lactamase inhibitors and enhancers to overcome resistance, including ceftazidime-avibactam, ceftolozane-tazobactam, cefepime-enmetazobactam, cefepime-zidebactam and cefepime-taniborbactam, and oral combinations such as ceftibuten-ledaborbactam are in development. Siderophore cephalosporins like cefiderocol use bacterial iron uptake systems to enter cells. These agents target carbapenem-resistant Enterobacterales and Pseudomonas.
• Emerging Market Innovation and Supply DiversificationIndian companies are moving from generics to innovation, with Wockhardt's cefepime-zidebactam approved in India and the United States in 2026 and Orchid Pharma producing cefepime-enmetazobactam. Governments are encouraging domestic production of cephalosporin active ingredients to reduce dependence on imports, including India's production-linked incentive scheme, and Western countries are considering stockpiling and diversified sourcing to prevent shortages.

Segment Analysis Third-generation cephalosporins account for the largest share of cephalosporin use and value, while newer generations and combinations are the fastest-growing segments. First-generation cephalosporins, including cefazolin, cephalexin and cefadroxil, are widely used for skin and soft tissue infections and are the standard antibiotic for surgical prophylaxis before many operations, making cefazolin one of the most used hospital antibiotics. Second-generation cephalosporins, including cefuroxime, cefaclor, cefprozil and cefoxitin, treat respiratory infections and some abdominal and gynaecological infections. Third-generation cephalosporins, including ceftriaxone, cefotaxime, ceftazidime, cefixime, cefpodoxime and cefdinir, account for the largest share of use, treating pneumonia, meningitis, sepsis, urinary tract infections, gonorrhoea and many hospital infections, with ceftriaxone among the most widely used antibiotics globally. Fourth-generation cefepime is used for serious hospital infections, including in patients with neutropenia. Fifth-generation or anti-MRSA cephalosporins, ceftaroline and ceftobiprole, treat MRSA infections, pneumonia and bacteraemia. Siderophore cephalosporins, such as cefiderocol, treat carbapenem-resistant Gram-negative infections. Cephalosporin and beta-lactamase inhibitor combinations, including ceftazidime-avibactam, ceftolozane-tazobactam, cefepime-enmetazobactam and cefepime-zidebactam, are the fastest-growing branded segment.

Injectable cephalosporins account for the largest share of cephalosporin value because of hospital use, while oral cephalosporins account for a large share of prescriptions in outpatient care. Injectable cephalosporins, given intravenously or intramuscularly, include cefazolin, ceftriaxone, cefuroxime, ceftazidime, cefepime, ceftaroline, ceftobiprole, cefiderocol and all the newer combinations, and they are used in hospitals for serious infections, surgical prophylaxis and outpatient parenteral therapy. Ceftriaxone's once-daily dosing makes it suitable for outpatient and home intravenous therapy and intramuscular treatment of gonorrhoea. Injectable forms account for the largest share of value because of higher prices, hospital use and branded agents. Oral cephalosporins, including cephalexin, cefadroxil, cefuroxime axetil, cefaclor, cefprozil, cefixime, cefpodoxime and cefdinir, are widely prescribed in primary care for respiratory, urinary, skin and ear infections, particularly in children, and are available as tablets, capsules and suspensions. Oral cephalosporins are especially common in Asia and Latin America. Development of new oral agents active against resistant bacteria, such as ceftibuten-ledaborbactam, aims to allow earlier discharge and outpatient treatment. Powder for injection and ready-to-use formulations are produced by specialised manufacturers.

Respiratory tract infections and urinary tract infections account for the largest share of cephalosporin use, while bloodstream infections, surgical prophylaxis and hospital-acquired infections are major hospital applications. Respiratory tract infections, including community-acquired pneumonia, bronchitis, sinusitis, pharyngitis and otitis media, are major indications for oral and injectable cephalosporins, and ceftriaxone is a standard treatment for hospitalised pneumonia. Urinary tract infections, including cystitis, pyelonephritis and complicated urinary tract infections, are treated with oral and injectable cephalosporins, and new combinations such as cefepime-enmetazobactam and cefepime-zidebactam were approved for complicated urinary tract infections. Skin and soft tissue infections are treated with first-generation and anti-MRSA cephalosporins. Surgical prophylaxis with cefazolin is used before most clean and clean-contaminated operations. Bloodstream infections and sepsis require broad-spectrum injectable cephalosporins, and ceftobiprole was approved for Staphylococcus aureus bacteraemia. Hospital-acquired and ventilator-associated pneumonia are treated with ceftazidime, cefepime, ceftolozane-tazobactam, ceftazidime-avibactam and cefiderocol. Intra-abdominal infections, meningitis, bone and joint infections, gonorrhoea and Lyme disease are further indications. Febrile neutropenia in cancer patients is treated with cefepime or ceftazidime.

Hospital pharmacies account for the largest share of cephalosporin value, while retail pharmacies dispense the largest volume of oral cephalosporin prescriptions. Hospital pharmacies supply injectable cephalosporins for surgical prophylaxis, serious infections and intensive care, and they are the main channel for newer branded cephalosporins and combinations, where antimicrobial stewardship teams control use. Hospitals purchase cephalosporins through tenders, group purchasing organisations and national procurement programmes. Retail pharmacies dispense oral cephalosporins prescribed in primary care, particularly for children's ear and respiratory infections and urinary tract infections, and in many low- and middle-income countries cephalosporins are sold without prescription, contributing to resistance. Online pharmacies have grown in some markets. Outpatient parenteral antimicrobial therapy services administer ceftriaxone and other injectables at home or in infusion centres. Government procurement and public health programmes, including sexually transmitted infection programmes, purchase ceftriaxone and other cephalosporins. China's volume-based procurement has reduced prices of many cephalosporins sold to public hospitals. Veterinary use of cephalosporins is regulated separately but affects resistance.

Regional Analysis Asia-Pacific accounts for the largest share of cephalosporin consumption and production, while North America and Europe generate high value from branded newer agents and hospital use. Asia-Pacific is the largest region for cephalosporin consumption and manufacturing. China and India produce most of the world's cephalosporin active ingredients and generic finished products, and both countries have high use of cephalosporins in hospitals and outpatient care. China's volume-based procurement has lowered prices for cephalosporins, while India's production-linked incentive scheme supports domestic manufacturing of key starting materials and ingredients. Wockhardt's cefepime-zidebactam, approved in India on 27 May 2026 and by the FDA days later, marked a milestone for Indian antibiotic innovation. Japan's Shionogi developed cefiderocol, and Japan has high use of oral cephalosporins. North America generates high value from newer branded cephalosporins such as ceftazidime-avibactam, ceftolozane-tazobactam, cefiderocol, ceftobiprole and the new combinations approved in 2024 and 2026, while generic cephalosporins are widely used in hospitals and shortages of cefazolin and other injectables have occurred. Europe has strong antimicrobial stewardship, lower use of broad-spectrum cephalosporins in northern countries and higher use in southern and eastern Europe, and the UK's subscription model pays for access to cefiderocol and ceftazidime-avibactam. Latin America, the Middle East and Africa have high cephalosporin use and rising resistance, often with antibiotics sold without prescription, and rely on imports from India and China.
